How often does The Joint Commission survey accredited hospitals?

Unannounced and roughly triennial — the survey can arrive any day.

Joint Commission hospital surveys are unannounced and occur approximately every three years — typically within an 18-to-36-month window after the previous survey. You won’t get a date. The operating assumption is simple: be survey-ready every day, because the surveyors can walk in any day.

How the unannounced cycle works

After your last survey and accreditation decision, the clock starts. The Joint Commission schedules the next full survey somewhere in the 18–36 month range but discloses nothing to the organization. Survey teams arrive without notice and typically spend several days on site, evaluating compliance across all applicable standards — clinical, environment of care, life safety, infection control, leadership, and medication management.

What can trigger a survey outside the cycle

The three-year rhythm isn’t the only way surveyors show up:

  • Follow-up surveys — an “Accreditation with Follow-up Survey” decision brings surveyors back within six months to verify sustained compliance.
  • For-cause surveys — complaints, sentinel events, or CMS referrals can trigger an unannounced focused survey at any time.
  • Initial surveys — new organizations seeking accreditation get scheduled (announced) initial surveys.

What “survey-ready every day” actually requires

It doesn’t mean living in a panic. It means the systems run continuously: EOC rounds happen on schedule with findings closed on time, testing and inspection logs are current, ILSM documentation is live for any active project, and staff can answer basic safety questions on any shift — including nights and weekends, when surveyors love to arrive. The organizations that dread surveys are the ones that prepare in sprints. The ones that don’t are the ones whose program never stops.

The survey is unannounced. Your preparation shouldn’t be.

FAQ

Will we get any warning before a Joint Commission survey?
No. Full accreditation surveys are unannounced. Only initial surveys for new organizations are scheduled.

How long does a hospital survey take?
Typically several days, depending on facility size and complexity, with a multidisciplinary surveyor team.

Does DNV survey on the same cycle?
No — DNV conducts annual surveys under its NIAHO/ISO 9001 model, a different rhythm from the Joint Commission’s approximately-three-year cycle.
